There are three ways to apply for WV HIPP benefits! You can:

1. Fill out and submit an online application,

2. Print a WV HIPP application and mail or fax your completed application, or

3. Call us toll-free at 1-855-MyWVHIPP (1-855-699-8447) to start your application over the telephone.

If you are enrolled in employer-sponsored insurance or wish to be, you will need to send in the following information before you can officially be HIPP eligible and receive payments:

• A copy of the front and back of your insurance card
• Summary of benefits for your policy plan or desired policy plan
• Your employer’s health insurance rate sheet
• A copy of your paystub that shows a premium deduction

If you have not yet enrolled in your employer’s health plan, you can start the application process by providing a rate sheet from your employer. HIPP will tell you what type of policy you may qualify for.

Per federal regulation, your employer must provide you group health coverage if you qualify for HIPP, regardless of whether or not their open enrollment period has passed.
